You can make any appointment with Sound Women's Care by calling our clinic at 425-640-4810 between the hours of 8:30 am and 5:00 pm. You may bring the completed forms with you to your appointment or fax them to us 425-640-4884 prior to your appointment. Of course, all patient information is kept strictly confidential in accordance with our privacy policy.

Please download and fill out the following forms (PDF format) before your appointment to save time:

» Established Patients Forms — Complete both the Registration and Established Patient forms if you have not been seen at Sound Women’s Care in the past 12 months, or are coming in for your annual exam.

» New Patient Forms — Complete both the Registration and New Patient Form if you are new to Sound Women’s Care or it has been 3 or more years since your last visit with us.

 

 Privacy Policy

Sound Women's Care does not collect personally identifiable information about individuals except when such individuals specifically provide such information on a voluntary basis. Such personally identifiable information may be gathered from transactional areas and information request forms. Personally identifiable information on individual users will not be sold or otherwise transferred to any third parties except to medical professionals for bona fide medical purposes.

Sound Women's Care reserves the right to perform statistical analyses of user behavior and characteristics in order to measure interest in and use of the various areas of its web site and adjust content accordingly.

Users also should be aware that non-personal information and data may be automatically collected through the standard operation of Sound Women's Care internet servers or through the use of "cookies." "Cookies" are small text files a web site can use to recognize repeat users, facilitate the user's ongoing access to and use of the site and allow a site to track usage behavior and compile aggregate data that will allow content improvements and targeted advertising.
