Patient Forms

You can make any appointment with Sound Women's Care by calling our clinic at 425-640-4810 between the hours of 8:30 am and 5:00 pm Monday through Friday. You may bring the completed forms with you to your appointment or fax them to us at 425-640-4884 prior to your appointment. Of course, all patient information is kept strictly confidential in accordance with our privacy policy.

Please download and fill out the following forms (PDF format) before your appointment to save time.

Established Patients

• Established GYN patients that are coming in for an annual exam or that have not seen us in the past 12 months please complete these forms and bring them with you to your appointment.
• Established OB patients and are coming in for obstetrical care please complete these forms and bring them with you to your appointment.

New Patients

• New GYN patient please complete these forms and bring them with you to your appointment.
• New OB patient please complete these forms and bring them with you to your appointment.

All new patients should also review our Notice of Privacy Practices and Patient Financial Policy and save a copy of each for your records.

Medical Records Release

Please complete this form for any records that need to come to our office from another provider. This form can also be used to forward your records to another office.
